Connecting Communities to the Right Support
Health, social care, emergency services, housing, councils, and the voluntary sector are all helping the same communities but with no shared way to connect people to the right support. Referrals get lost. People tell their story over and over. Early intervention opportunities get missed.
I built a digital referral platform - one shared pathway for professionals and residents to search, refer, and track. The focus is on warm referrals: not just sending information into the void, but making connections that actually lead to outcomes. Professionals from any partner organisation can search over 2,700 local services, make a referral in under a minute, and track what happens next.
The self-referral pathway means residents can find and access support directly too - no gatekeeper, no waiting for someone to make a phone call on their behalf. And the data layer gives commissioners real visibility into what's happening across their patch: where demand is, what's working, where the gaps are. It's the kind of platform that quietly becomes essential infrastructure - the thing nobody notices until it's not there.
